ISP Supplemental Questionnaire This form is required for international students who completed the domestic CCCApply application for admission.  ISP
ISP Document Upload Use this form to submit supporting documents for your application to SRJC (passport, bank statements, English proficiency, etc.) ISP
Student Enrollment Reservation (SER) Admitted students can submit this form to accept their offer of admission and save their seat in the upcoming class.  ISP
Online Orientation Evaluation New/returning students are encouraged to provide anonymous feedback about the online orientation experience.  N/A
Travel Authorization Request For a travel signature on your Form I-20 allowing you to re-enter the U.S. Complete this form BEFORE you depart the U.S.  ISP
Reduced Course Load (RCL) Request to drop below 12 units. Allowed for academic difficulty in the first term, medical reasons at any time, or final term.  ISP, and Counseling or Student Health Services
Program Extension If you need additional time to complete your program of study, request this BEFORE your Form I-20 expires.  ISP and Counseling
Transfer-Out SRJC international students who have been accepted to attend another U.S. school may request a transfer of their SEVIS record.  ISP and new school
Leave of Absence (LOA) International students at SRJC who need to return to their home country for a semester may request a leave.  ISP and SEVP
Authorized Early Withdrawal For students who need to withdraw from SRJC before the Program End Date on their Form I-20. ISP
Curricular Practical Training (CPT) Students who wish to gain professional experience in a job related to their major can apply for CPT authorization to work during their studies. ISP, Work Experience/Internship Coordinator, and Department Chair
Optional Practical Training (OPT) Students who wish to gain professional experience in a job related to their major can apply for OPT authorization to work after their studies.  ISP and USCIS
Economic Hardship Employment Authorization Students who are experiencing financial difficulties due to circumstances beyond their control may apply for this off-campus work authorization.  ISP and USCIS
Reinstatement Students who have fallen out of F-1 status may request reinstatement to regain their legal status in the U.S. ISP and USCIS
International Club Sign-Up For currently enrolled students who want to join the SRJC International Club.  ISP
